Health Manager
Job Summary
Collaborates with Director of Home Health Services in planning, developing
and implementation of the Agency Home Health Program; including philosophy,
policies, goals and objectives. Supervises the coordination
of Home Health services and provides supervision of the clinical
staff. Ensures compliance with Medicare Conditions of Participation,
quality improvement and compliance with the mission and services
of the Agency.
Responsibilties & Duties
-
Monitors patient scheduling to
assure that all requests for service
are responded to in a timely and
professional manner and are assigned
appropriately.
- Confirm that patient/families are
provided with options or creative
solutions for those who present with
unique or atypical needs.
- Collaborates with scheduler, referral
sources, etc. as needed and offers
clinical consultation on patient
care issues.
- Ensure compliance with CHAP conditions of Medicare participation
and Title 22 requirements.
- Periodically reviews clinical documentation to ensure compliance
with documentation standards and proper utilization of services.
- Participate in skills assessment process by conducting annual
competency visits for staff members and/or delegates as appropriate.
- Verify that team conferencing is conducted as required by Agency
policy and procedure.
- Monitor staff productivity to ascertain that established guidelines
are achieved.
- Review productivity on a weekly basis.
- If productivity requirements are not met, initiate coaching and
counseling with the employee.
- Collaborates with the Orientation/Education department to determine
ongoing educational needs of team members and implement education
plan.
- Participates in the Home Health Quality Improvement Program to
identify areas of concern.
- Assist in the development of plans of correction for identified
areas.
- Integrate recommendations into the staff’s daily functions.
- Evaluate and maintain appropriate staffing levels.
- Determine required staffing levels to provide quality patient
care.
- Participates in the recruiting, interviewing and hiring process.
- Promote open communication with
staff
- Communicate
policy and procedure changes within
the department, including the reasons
for the change.
- Disseminate
information regarding the Agency’s
financial status, personnel changes,
updates on new programs, etc. on a
regular basis.
- Create ongoing opportunities for
employee to share feelings, experiences,
concerns and make suggestions.
- Evaluate staff performance on a
continuous basis
- Conduct job performance reviews
for team members on a timely basis.
- Provide regular feedback regarding
job performance.
- Any performance issues need to
be addressed at the time of occurrence
and in a consistent manner throughout
the Agency
- Coach, counsel and discipline
as appropriate. Inform Senior
Management and Human Resources if
serious disciplinary action is warranted.
- Foster collegial staff–management
relationships.
- Exhibit mutual respect for each
other’s knowledge and concern
for the provision of quality patient
care.
- When problems and/or issues are
raised, address the issue in a timely
manner. Whenever possible,
the proposed solution should be reached
collectively and must be communicated
with everyone as quickly as is reasonable.
- Maintain a high level of customer
satisfaction with contracted nursing
facilities, hospitals, referring
physicians, Managed Care organizations
and private insurance companies.
- Resolve all complaints in a prompt
and professional manner.
- Review and complete incident reports
and service complaints, consistent
with Agency policy.
- Notify Senior Management immediately
of any potential risk management
issues.
- Maintains positive relationship
with all customers.
- Maintains excellent communication
with internal employees.
- Participate in community education
efforts to promote the home health
philosophy of care and to improve
general community and professional
understanding of home health.
- Participates in physician office
educational lunches when required
as appropriate
- Participates in health fairs as
appropriate.
- Attends various hospital and skilled
nursing facility meetings as necessary.
- Seek out opportunities for improving
services, implementing new products,
policies and procedures.
- Remain abreast of new products
available within the industry.
- Keep current regarding the latest
procedures and treatments applicable
within the Home Care field.
Reporting Relationships:
The Home Health Manager reports
directly to the Director of Home
Health Services.
Specifications:
Knowledge, Skills & Abilities:
- Current California RN license.
- Current CA Driver’s License.
- Strong knowledge of current clinical
practices with minimum of 5 years
in nursing and 3 years of home
health/hospice experience preferred.
- Minimum of one (1) year supervisory/management
experience required.
- Excellent interpersonal, verbal
and written communication skills
- Ability to establish goals and
monitor progress against goals.
- Ability to handle multiple projects
at the same time with little supervision.
- Ability to make quick, sound
decisions utilizing critical thinking
skills
Physical Requirements:
- Body Positions: Sitting and standing for prolonged periods.
- Body Movements: Arm and hand dexterity; ability to drive an automobile.
- Body Senses: Must have command of close and distant sight, color
perception and hearing.
- Strength: Ability to lift and move
up to 25-pounds.
Working Environment:
- Work in an office, where the climate is controlled.
- Travel to various local offices, i.e. physician office, hospital,
for meetings.
- Travel by car to patients’ homes, where the employee does
not have control over how the climate
is regulated.
- OSHA exposure category:
Category I – Position includes tasks
that involve exposure
to Bloodborne Pathogens.
Category II – Position includes tasks
that do not have exposure
to Bloodborne Pathogens; however,
employment may require unplanned
Category I tasks.
Category III – Positions includes
tasks that do not involve
exposure to Bloodborne Pathogens. This
position would not be required to perform
Category I tasks.
