Speech & Language Pathologist
Job Summary
Under the supervision of the Rehabilitation Manager, provides speech & language therapy services for patients who have speech, language or swallowing problems.
Responsibilities and Duties
- Provide speech therapy care to patients within accepted standard of practice and Agency policy and procedure.
- Perform an initial diagnostic speech and language and/or swallowing assessment, including the Outcome and Assessment Information Set (OASIS) assessment.
- Develop a care plan, which establishes goals, based on the patients’ identified problems and incorporate interventions designed to improve the patient’s speech/language/swallowing ability. Include patient and the family in the planning process.
- Perform ongoing assessment of the patient’s speech, language and/or swallowing abilities and needs, updating the plan of care as necessary. Evaluate the effectiveness and outcomes of care.
- Provide interventions based on speech and language therapy standards and clearly defined treatment protocols.
- Inform the physician and other staff of changes in the patient’s needs.
- Coordinate services with other healthcare providers, including medical social worker, nursing services, other rehabilitation services and community resources as appropriate.
- Complete all documentation in accordance with regulations and Agency policy and procedure.
- Maintain Agency productivity guidelines
- Actively collaborates with team members in establishing, maintaining and updating an individual written plan of care for each Home Health patient.
- All treatment protocols and interventions are outlined in the plan of care and are consistent with the philosophy, mission and purpose of the organization.
- The plan of care is utilized as a guide for all patient care visits and is updated per Agency policy.
- Review outcomes and barriers to care. Set new goals with achievable outcomes based upon team input.
- Coordinate with other team members on planning discharge from speech therapy service. Ensure that all necessary steps are taken and appropriate documentation is completed.
- Provides ongoing education and consultation regarding speech therapy.
- Educate the patient/family regarding speech, language & swallowing interventions and the process for resolution of care and complaint issues.
- Promote patient independence.
- Collaborate with other staff members regarding patient needs and issues as necessary.
- Participate in the Home Health Quality Improvement Program.
- Assist in identifying areas of concern.
- Assist in the development of plans of correction and integrate recommendations into the staff’s daily functions.
- Participate in chart audits at Utilization Review meetings or as directed by Manager.
- Complete incident reports for adverse events in accordance with Agency policy.
- Participate in community education efforts to improve general community and professional understanding of home health services.
- Participates in education of skilled nursing facilities and other community health care providers.
- Present in-services upon request.
- Maintain a high level of customer satisfaction.
- Resolve any complaints that fall within scope of authority in a prompt and professional manner. Refer the complaint to a Rehabilitation Manager or the Home Health Director as appropriate.
- Complete service complaint reports, consistent with Agency policy.
- Maintain positive relationship with patients, families, referral sources, the general public and staff.
- Conduct all communication with internal and external customers in a professional manner.
- Maintain excellent verbal communication with staff and management team.
- Participate in Agency staff development training.
- Attend all required in-services pertaining to job responsibilities.
- Complete Agency mandatory annual in-service training.
Reporting Relationships:
The Speech & Language Pathologist reports directly to the Home Health Manager.
Specifications:
Knowledge, Skills & Abilities:
Possess a current Speech & Language Pathology license in the State of California.
Certificate of Clinical Competence from the American Speech & Hearing Association.
Possess a valid California drivers’ license.
Strong clinical skills required, with a minimum of one (1) year speech therapy experience in an acute or rehabilitation setting. Prefer at least two (2) years of speech therapy with one year of Home Health experience.
Knowledge of regulatory requirements: Title 22, Medicare Conditions of Participation.
Above average interpersonal and verbal communication skills.
Ability to establish goals and monitor progress against
goals.
Ability to make quick, sound decisions, utilizing critical thinking skills.
Possess strong public relations skills.
Ability to handle multiple assignments at the same time with limited supervision.
Physical Requirements:
Body Positions: Sitting, standing and driving.
Body Movements: Arm and hand dexterity.
Body Senses: Must have command of close and distant
sight, color perception and hearing.
Strength: Ability to lift and move up to 20 pounds.
Working Environment:
Partial time spent working in an office, where the
climate is controlled.
Travel by car to patients’ homes, where the employee
does not have control over how the climate is regulated.
OSHA exposure category:
Category I – Position includes tasks that involve exposure
to Bloodborne Pathogens.
Category II – Position includes tasks that do not have exposure to Bloodborne Pathogens; however, employment may require unplanned
Category I tasks.
Category III – Positions includes tasks that do not involve exposure to Bloodborne Pathogens. This position would not be required to perform Category I tasks.
